Personal Emergency Response Systems (PERS)
Senior Smart Personal Emergency Response Systems are designed to provide fast, reliable access to help—whether your clients are at home or on the go. A Personal Emergency Response System (PERS), also known as a medical alert system, allows clients to quickly summon help in the event of an emergency. Senior Smart PERS consist of a wearable transmitter, such as a pendant or bracelet. When activated, the transmitter alerts the monitoring center who immediately notifies designated contacts and/or dispatches emergency services, depending on the subscriber’s preference.
Environmental Assessments
An Environmental Assessment is conducted for an individual with a disability who may be experiencing challenges in maintaining safety and independence at home. The primary purpose of the assessment is to identify home safety and accessibility concerns and to recommend appropriate solutions.
The process includes an interview with the client or the client’s representative to gain insight into the client’s abilities, health conditions, fall history, family support, and other relevant factors. The assessor then conducts a walkthrough of the home to assess the environment and identify potential risks or barriers.
At the conclusion of the assessment, all recommendations are discussed with the client or client’s representative. The assessor will then submit a comprehensive report to the assigned Support Planner for review. The Support Planner will then advocate for the implementation of the necessary home modifications.
